Custodian Declaration Form (IMM 5646) – Notarization in Vaughan

If your child is coming to Canada as an international student under the age of 18, the Canadian government requires a notarized Custodian Declaration Form (IMM 5646). This form designates a responsible Canadian adult (the custodian) who will care for and support the student while studying in Canada.


Why IMM 5646 Is Required?

IRCC mandates the IMM 5646 for minor students to ensure that a trusted adult is responsible for them during their stay. This document:


  • Confirms the custodian is a Canadian citizen or permanent resident
  • Shows the parent/guardian’s consent for the custodian to act on their behalf
  • Provides legal assurance that the minor will have adequate care in Canada
  • Must be notarized by both parties for IRCC acceptance

Who Needs It?

You’ll need to notarize IMM 5646 if:


  • The student is under 18 (or under 19 in some provinces)
  • They’re coming to Canada without a parent or legal guardian

The custodian is not a family member traveling with them

Notarization Requirements


To complete IMM 5646 notarization:


  • The parent/legal guardian must sign their portion in front of a notary
  • The designated custodian must sign their portion in front of a notary
  • Each side must include full names, addresses, and signatures
  • A valid ID must be presented for both signers
