Standard Format of an Ontario Affidavit
Here’s a breakdown of what a basic affidavit typically includes:
1. Title:
“Affidavit of [Your Full Name]”
2. Introductory Statement:
“I, [Full Name], of the City of Vaughan, in the Province of Ontario, make oath and say as follows:”
3. Statement of Facts:
- Point 1: Clear and simple statement
- Point 2: Add additional facts line by line
- Point 3: Avoid emotional or irrelevant language
4. Signature Block:
Signed and sworn/affirmed before a notary

- Signature and stamp of notary
